                   Medical coding is the transformation of healthcare diagnosis, procedures, medical services, and equipment into universal medical alphanumeric codes. The diagnoses and procedure codes are taken from medical classification systems, such as the International Classification of Diseases (ICD) and the Healthcare Common Procedure Coding System (HCPCS).  


                 The rules and regulations of medical coding are based on the guidelines established by the American Medical Association’s Current Procedural Terminology (CPT) codebook and the International Classification of Diseases (ICD) codebook. Medical coders are required to adhere to the rules and regulations of the coding system in order to accurately record medical services and diagnoses. 

These rules and regulations include the following: 

1. Codes must be selected from the appropriate coding system. 

2. Each code must be sequenced in the correct order. 

3. Each code must be supported by appropriate documentation. 

4. Codes must be applied accurately and consistently. 

5. Codes must be updated in accordance with the coding system. 

6. Coding must be done in a timely manner. 

7. Codes must be submitted to the appropriate payer. 

8. Codes must be assigned based on the patient’
